Print the document, fill in the requested information and add your signatures and dates. Check that every page is readable.
Upload your signed copy
Use the email address you used to enroll. Send a PDF containing all pages, or a clear JPG or PNG photograph.

Required: full name, enrollment email and your signed agreement. Documents can be opened only by academy administrators.
